Confined Space Entry Authorization---Implementation of revised format

 'Safe Work Practices on Confined Space Entry'.

Please note that regarding the document revision, the format of 'Confined Space Entry

Authorization' is revised and action has been initiated as follow:

 Printing the revised CSE Authorization forms.

 Incorporating the revised CSE Authorization form in the electronic work permit

system.

Upon completion of the above two actions, we will communicate the date of implementation

of the modified CSE Authorization format.

All other changes / additions (except for the format of CSE Authorization) specified in

revision # 4 of the document 'SWP on Confined Space Entry' will come into force with effect
